MannKind Edges Past EPS Consensus, but a Revenue Miss and an Implied Furoscix Shortfall Temper the ReadyFlow Launch

MannKind reported a third-quarter loss of $0.07 per share, a penny better than the -$0.08 consensus and a 12.5% positive surprise. The top line told a less flattering story. Revenue of $101.87 million rose 24.0% year over year but missed the $105.56 million consensus by 3.5%. Earnings fell 333.3% from a year ago, a swing from a $0.03 profit to a loss. Revenue growth also slowed sharply from the prior quarter's 43% pace. That is the central tension. The launch metrics behind the new products look genuinely strong, but the near-term financial trajectory has slipped. That includes Furoscix, which management has repeatedly held up as transformational.

The quality of the quarter deserves scrutiny in both directions. The GAAP net loss widened to $23.2 million from $19.0 million in the second quarter. However, $11.6 million of that was contingent-consideration expense tied to the ReadyFlow approval and the related CVR. On a non-GAAP basis the loss was a much smaller $0.01 per share, though that still compares poorly with $0.07 of adjusted income a year earlier. The cost base has fundamentally reset. SG&A more than doubled to $61.4 million on Furoscix promotion, field-force expansion and two simultaneous launches, and interest expense jumped to $7.6 million on the term loan. Sequentially, revenue fell from $109.4 million to $101.9 million, mostly because collaboration and services revenue dropped to $19.4 million from $35.0 million. Management attributes that to delivery timing on United Therapeutics orders and still expects the line to be flat for the full year. Royalties were the steadiest engine, rising to $37.5 million, up 16% from the second quarter and 12% year over year, as Tyvaso DPI returned to growth.

Underneath, demand clearly improved. ReadyFlow began shipping in mid-September and accounted for roughly 75% of Furoscix shipments by late October, a conversion pace management said is several quarters ahead of plan. The launch has drawn more than 1,600 prescribers, mostly with new-to-brand patients. Management has previously said ReadyFlow carries a materially better margin than the on-body infusor, which the release blames for the current gross-margin dilution. Afrezza revenue climbed to $19.3 million from $17.0 million in the second quarter, total prescriptions rose 17% sequentially, and new-to-brand prescriptions jumped about 80%. The pediatric launch reached all 60 priority institutions and roughly 2,000 patients.

The problem is that Furoscix revenue was essentially flat at $22.4 million versus $22.2 million, with about $3 million lost to the transition. Management now expects fourth-quarter Furoscix revenue to exceed the first half's total, roughly $38 million. That implies a full year near $98 million, below the $110-120 million target reaffirmed on the previous three calls. The earlier talk of a $450 million-plus 2026 revenue run rate also went unrepeated. Other soft spots:

  • Adult Afrezza underperformed amid field turnover, competition from type 2 insulin pumps and a much lower share of voice.
  • The pediatric $35 cash-pay program ends at year-end, and prescriptions are currently running ahead of revenue.
  • Ralinepag milestones of about $15 million slipped into 2027.
  • The Phase 2 nintedanib DPI readout is now guided to the first half of 2028.

The offsetting catalyst is significant. Management introduced guidance for GAAP and non-GAAP profitability in the fourth quarter and for full-year 2027, a marked shift from the earlier "investment year" framing. That commitment matters for a company with $96.6 million in cash and investments, a $319 million term loan and a stockholders' deficit. The $50 million private placement that funded the CVR payment also lifted the weighted share count to about 320 million. Bears have legitimate ammunition in Medicare deductible resets that management expects to weigh on first-quarter Furoscix and in planned 2027 sales-force spending. Both will test that profitability pledge.

Investor sentiment improved meaningfully heading into the report, moving from slightly negative at -0.07 to modestly positive at 0.26. The stock's path has been volatile. Shares peaked at $4.40 in mid-August, slid to $2.90 by mid-October, then rebounded nearly 36% to $3.93 ahead of the release. That leaves them only 2.9% above the open after the prior report and about 5.8% above the $3.71 200-day moving average. Price and AVWAP trend readings remain negative even as momentum has turned positive, which fits a stock still repairing damage. A sharp pre-report rally plus improved sentiment means expectations were rebuilt quickly, and a revenue miss with an implied guidance shortfall is not what that setup wanted.

The bottom line is that MannKind delivered a narrow EPS beat and launch metrics that look better than feared, but missed on revenue, decelerated sharply and effectively walked back its Furoscix target. The call preserved a credible growth story built on ReadyFlow conversion, pediatric Afrezza and Tyvaso royalties. The investment case now hinges on delivering the promised fourth-quarter profitability.
